Certain fixed-dose combination drugs which were in use in the Indian market has now been banned. If you are a doctor or a patient, you need to be aware of this list of drugs, which are banned.

How is a New Drug approved in India?

New Drug Approval the steps

  • At the Central level DCGI, under Ministry of Health and Family Welfare approves drug or Medical device for marketing.
  • But the manufacturing license is approved at State level by State drug control authorities.
  • Pharmaceutical companies often market a drug under their label, while manufacturing is done by another company.
  • Monitoring is done by state agencies coordinating with Central departments.

While most doctors avoid fixed-dose combination, drugs this list of banned drugs will prevent over the counter unscrupulous selling of many medicines, which are often bought and sold without a doctors prescription.

What are FDC drugs?

FDC or fixed-dose combination drugs have two or more active components.

What is the worry with FDC drugs?

  • The irrational use is giving rise to antibiotic resistance against infectious diseases.
  • Unnecessary extra drug intake causing side effects.
  • Inadequate dosage of drugs in some cases.
  • Prescriptions by unscrupulous Quacks, who lack knowledge about responsible drug therapy. Quacks are often unable to know the exact cause, effect relation of diseases and give combination drugs hoping one would work.

When it’s a combination drug, since its given together in a molecule, the pharmacological effect of it may be changed from what is known about each of them individually. In case of adverse reactions, its difficult to identify culprits

In the USA there are about 500 FDC drugs. In India, this number is more than 2000!

Going forward the responsibility lies with

  • The Government
  • Pharmaceutical Companies
  • Doctors and
  • People for the responsible use of medicines

Always visit your doctor or nearest Government healthcentre in case you are ill. Depending on OTC(over the counter) drugs prescribed by an unqualified person, puts you at unnecessary health risks.

Meningitis in Children:Symptoms,Signs & Dangers (Quick Guide)

When you think of all the things that cause a headache it can be mind-boggling. But children complaining of a headache with fever is unusual and can be cause for worry. That’s why as a parent there are certain things you should be aware of and take necessary protection. In the event of World Meningitis day, this is my way of helping spread the right words about meningitis and how you can prevent it.

10 Meningitis Facts That Every Parent Should Know

1)What exactly is meningitis?

Meningitis is an inflammation of the covering membranes which protect the brain and spinal cord. The cerebrospinal fluid surrounding the membranes may be infected.This causes membranes to become infected and inflamed.The protective membranes get weak and porous.

This disease can affect both children and adults. But the main problem in children is they are not able to complain and their system is not strong enough if adequate preventive protection not given.

2)Symptoms and Signs of Meningitis

Meningitis is a serious condition which can cause a headache accompanied by fever in children.

Important: Fever, poor feeding, neck rigidity are early warning signs of meningitis. The young child is irritable and may cry constantly.

A headache if in association with fever in a young child should be urgently assessed by a doctor. But often less than five years child may not even complain of headache, and that’s why you need to know other symptoms.

Other symptoms of meningitis include:

  • General poor feeling, sudden high fever
  • Poor feeding
  • Skin rash
  • Severe, persistent headache Neck stiffness(early warning in infants and  babies)
  • Nausea or vomiting
  • Discomfort in bright lights
  • Blurring of vision
  • Double vision
  • Eye pain
  • Characteristic posture
  • Seizures
  • Drowsiness or difficulty in awakening
  • Joint pain
  • Confusion or other mental changes

3)What are the main types of meningitis?

Most commonly

  • Viral
  • Bacterial

4)Causes of Meningitis

Viral: Mumps, measles, enteroviral, chickenpox, herpes, and many others.

Viral meningitis is the most common and is less severe than bacterial meningitis. Bacterial Meningitis is a serious condition with the potential for serious complications, death, and disability in survivors.

Bacterial Meningitis causes

  • Hemophilus influenzae type B
  • Pneumococcal
  • Tuberculous
  • Meningococcal

5)Why do healthcare professionals worry?

  • It’s often difficult to identify the exact bacterial cause of meningitis, till test results are available just from clinical features. The meningococcal rash is characteristic but some viral meningitis too may have rashes.
  • It’s a fast progressing condition and needs urgent intervention and hospitalization.
  • High risk of complications, disability, and mortality in susceptible.

The good news

There are vaccines available.

The HIB vaccine is included in the National Immunization schedule and Pneumococcal Vaccine is often given according to the IAP schedule.BCG vaccination included in the national Immunization schedule gives 50 to 80% protection against tuberculous meningitis.If unvaccinated, untreated case of tuberculosis may be complicated by meningitis.

5)Who is at risk for Meningococcal Meningitis diseases?

  • Meningococcal vaccination is recommended in special category cases like before cochlear implant surgery, before splenectomy
  • Nephrotic syndrome
  • Hepatic failure
  •  SLE
  • Immunodeficiency due to complement deficiency, HIV, others
  • Traveling to a country with a hyperendemic or epidemic of this disease
  • At-risk during a community outbreak.

6) How does Meningococcal disease spread?

The problem is meningococcal infection is highly infectious. Spreads through cough and cold droplet infection. Has a rapid progression and can be life-threatening in small children.

7)What determines the course of disease and fatality?

Environment, host factors, bacterial factors, and prevention of disease steps all determine the course of the disease. The serum level of Bactericidal antibody has an inverse relationship with the immunity against meningococcus. It’s highest in newborn and adults and the lowest between 6months to two years of age. Maximum mortality hence occurs in this age group and is mainly due to Immunologic inexperience. The body’s defense mechanism at that age group is unable to recognize this dangerous bacteria for what it is.

8)Immunity develops to some extent through the asymptomatic carriage of pathogenic and nonpathogenic meningococci and of antigenically related species. Smokers and those exposed passively to secondary cigarette smoke have a poor nasociliary immune mechanism and are at risk. Neisseria meningitides is a gram-negative cocci and is a commensal in1 0% of human population nasopharynx

9)What do you need to know if your child is not able to express his feelings?

Infants and young children: High-grade fever feeding difficulty and neck stiffness. Child cry is persistent and may become weak.

Nausea and projectile vomiting may be characteristic of signs of raised intracranial tension. Older child above five years will be able to complain about a headache and fever.

10)What can you do as a parent to prevent this deadly disease?

a)Check BCG vaccination was given or not(usually given at birth).

b)Check HIB vaccination was given or not: Usually given as a pentavalent vaccine along with DPT.

c)Talk to your Pediatrician about the Pneumococcal vaccine.

d)Talk to your Pediatrician about the need for the Meningococcal vaccine.

In case you are unsure about your child’s need for protection and vaccination consult your pediatrician. Be aware, stay protected stay healthy.

Ayush doctors practicing surgery in modern hospitals? What does MCI say about it?MCI says No!

This was originally written in 2018 and is updated according to current state in Nov 2020.

A burning question being debated everywhere: Can Ayurveda practitioners perform surgeries? They can according to Ayush, but they may not according to Medical Council of India. While the debate rages, one thing you must remember, this is not a short-term problem. The crisis of doctors and bursting population needs long-term planning, Contraception(for population management), training of quality doctors as modern medical practitioners plys good policy making.

In this article, I am putting forward the views curated from the authoritative sources plus my own takeaway as a Modern medical practitioner and Indian citizen.
This is what Ayush Ministry said:

Those trained to perform surgeries can do it everywhere, says Ayush ministry. In answer to a Central Information Commission order in June asking the Ayush ministry “to clarify upon the issue of entitlement and extent of practitioner of India system of medicine to perform surgeries lawfully”, the ministry said surgeries were a part of Ayurveda curriculum.

“They can perform whatever is in their syllabus, but there are obviously certain exceptions. They cannot perform all procedures, including certain major surgeries. For more clarity, we are drawing up a list of do’s and don’ts so we can convince the MCI,” said Vaidya Rajesh Kotecha, secretary Ayush.
This is what the Medical Council of India said

Medical education regulator Medical Council of India (MCI) clearly says Ayush practitioners cannot do surgery in hospitals for modern medicine. (Reference)

Ayush ministry sought MCI approval, which was refused.

“We have refused as under MCI rules, crosspathy (mixing different methods of medicine) isn’t allowed. MCI’s stand is quite clear on this; Ayurveda surgeons cannot be mainstreamed. Surgery and associated procedures are part of modern medical science; whatever these practitioners are taught as part of their curriculum, cannot be allowed to be practices in allopathic hospitals,” said Dr Reena Nayyar, secretary, MCI.

This is what the Chairman of the regulatory body for Ayurveda, the Central Council of Indian Medicine (CCIM), Vaid Jayant Deopujari, tells that there is nothing new about ayurvedic doctors doing surgeries. He also mentions about the lack of assessment reports of Ayurveda hospitals is due to a loophole in the law.

What Parliamentary Standing Committee reviewing National Medical Commission Bill said

In March, the Parliamentary Standing Committee reviewing the National Medical Commission Bill, 2017, also recommended against Ayush doctors practicing allopathy after completing a short-term course.

Dr. Mukhtiar Chand & Ors. Vs. State Of Punjab & Ors., decided by the Supreme Court on 08/10/1998, reported as AIR 1999, SC 468, (1998 (7) SCC 579) K.T. Thomas, Syed Shah Mohammed Quadri,” A harmonious reading of Section 15 of 1956 Act and Section 17 of 1970 Act leads to the conclusion that there is no scope for a person enrolled on the State Register of Indian medicine or Central Register of Indian Medicine to practise modern scientific medicine in any of its branches unless that person is also enrolled on a State Medical Register within the meaning of 1956 Act.”

Takeaway:

No one has ever refused Ayush Doctors permission to practice Ayush and operate according to their experience. Let them heal with their own science.I am sure it will benefit all those who seek it.

Let the Ayush practitioners do surgery in Ayush hospitals with full responsibility. Any person who willingly gives informed consent for such procedures is doing so voluntarily.No such procedures should be done under the cover of modern medical services. Also, anesthesia, postop, further evaluation should be done under Ayush practitioners only. Provisions of medical insurance and all responsibility should lie with Ayush Hospital and the Ayush doctor. That will be in the best interest of all concerned.

This helps in transparency and better decision-making for doctors, patients, and all concerned.

Developing Ayush is a good thing. They have their strengths, let them enhance that. If they are qualified to practice modern medicine, they should pass all the exams of modern medicine too.No shortcuts work when it’s your health.

Who knows one day Ayush will regain its former glory. But this cannot be done piggybacking on cross pathy.

It’s also important that for the sake of better patient care assessment records of Ayush hospitals be made accessible.

There’s no one-size-fits-all in healthcare. The vast population needs quality care. There’s a reason for entrance exams and screenings. When you ignore that for short-term reasons, everyone suffers. I wrote this in 2018 and even in 2020 this continues. You need to be aware.

Bridge course for Ayush Doctors available! Would you like to get treated? Do you think death and diseases differentiate between rich and poor? Do you think the rural poor deserve worse healthcare?

Do you think the middle class deserves to be treated by quacks? Do you feel healthcare should be accessible and quality for everyone?

Do you feel you deserve to know what will happen to you if you visit any tier 2,3 cities towns and villages? As a Medical teacher in a small town in India, should I stop living here and treating poor patients? Should they be destined to be treated by Ayush doctors with a crash course in medicines and given a license to treat patients? In case a complication arises due to such treatment, will the Government take full responsibility for medical negligence?

If the decision rests with the State Government, will the state Government pay damages for disability, death, and negligence? I have spent nineteen years studying and still study, this will be taught in six months? As a doctor, I know this is serious. If there is a reason for the stringent eligibility tests for pre-medical and post-grad medical seats, why such a fish-eyed view about the health care of the masses?

Three things you should know as a rare Indian

1)If you are reading this post, that means you are an educated Indian and have above average IQ and income.

2)You already have access to quality health care and you have never been treated by a Quack.

3)If you don’t stand up to this health care crisis, a day will come when politicians will ensure only they get treated by Doctors with Medical degrees and all others by bridge course doctors. Not a Doomsday theory just simple logic.

Why is the Government doing this?

  • There is a real Crisis of Doctors.
  • A high attrition rate of Doctors.
  •  Challenging health care infrastructure, manpower, and bursting population.
  • Inability to think straight due to air, water, sound, and food pollution.
  • Brain suffering from Low oxygen saturation in capital city Delhi?
  • The Government think tanks are being misled by people with a vested interest?
  • The Government really wants to solve the healthcare crisis.
CAN HOMEOPATHY Practice Allopathy?

The answer is for them to know and you to guess.

If you believe in karma then you will know all those who are poor will not always stay poor. The tables may turn one day. Why endanger poor people?

These bridge course doctors will be placed in Midlevel care providers in sub-health centers. This means people who are poor will be denied quality healthcare, have poorer health, spend more money on controversial health advice and suffer eventually. Those who need help most will suffer most. Sigh!

Mainstreaming Ayush? This step is unlikely to help. It will only increase distrust and cause this ancient system to lose goodwill. Because Ayush has a role in Holistic wellness which makes it unique.

If mainstreaming Ayush is the intention, why not setup Ayush subcentres and let people visit those of their own free will and get treated. Why not open Ayush Hospitals with Ayush nurses, physicians, and surgeons. Whatever it is, gambling with the health of the nation needs someone to take responsibility. The Centre has very smartly passed the ball to the State Government, now who will roll the dice?

Cabinet removes Bridge course for Ayush Practitioners.

Instead, the responsibility of promoting and meeting the need for healthcare in the States has been delegated to the State Government. So if the question now is can Ayush doctors practice allopathy? The answer is still no for most parts of India. But in a few states, the State Government for some strange reason allowed this.

Can Homeopathy Practice Allopathy?

The answer is No.

Can BAMS practice allopathy or allopathic medicine?

Answer: NO.

Meningitis Facts You Can’t Afford to Ignore as a Parent #IMDAware

When you think of all the things that can cause a headache it can be mind-boggling. But your child complaining of a headache with fever is unusual and can be cause for worry. That’s why as a mother and a Doctor I am always trying to stay up to date with healthcare information.

And so I am taking this opportunity to help spread the right words and create awareness about meningitis and how to prevent it.

Do you know about meningitis?

It’s a cause for worry especially for infants and young kids. It causes an inflammation of the covering membranes that protect the brain and the spinal cord.

As my daughter was growing up, I kept a mental checklist of danger signs. Being a doctor, it was easy for me. But as parents, it is our job to be vigilant for signs that could mean more than simple flu or tantrum. Today I will share with you the warning signs for meningitis you must keep a lookout for. Fever, poor feeding, and neck rigidity are early warning signs of meningitis.

Other symptoms of meningitis include:

  1. General poor feeling, sudden high fever
  2. Skin rash
  3. Nausea or vomiting
  4. Discomfort in bright lights, blurring of vision, double vision and/or eye pain
  5. Seizures
  6. Drowsiness or difficulty in awakening
  7. Joint pain
  8. Confusion or other mental changes

The good news

There are vaccines available. My daughter received BCG, HIB, and pneumococcal and after researching about a meningococcal vaccine that’s next in line.

Is it contagious?

Yes, it’s highly infectious. It can spread through cough and cold droplet infection.

With reported cases of meningococcal meningitis increasing in India and no routine immunization, it’s making me feel unsafe as a mother. When I know it’s highly infectious and when I know prevention is possible I don’t want to take a risk. Prevention is better than cure any day!

What I did as a parent, to prevent this deadly disease?

  1. Check whether BCG vaccination was given or not (usually given at birth): given to my daughter.
  2. Check HIB vaccination was given or not (usually given as a pentavalent vaccine along with DPT): given to my daughter.
  3. Talk to your Pediatrician about the Pneumococcal vaccine: my daughter received it.
  4. Talk to your Pediatrician about the need for the Meningococcal vaccine.
