Painless vaccine for Whooping Cough?
Does this work?Are there painless options for everyone?The answers get interesting.Vaccine and painless? Sounds like an answer to a prayer. Who in their right mind would like to cause pain to babies right?

Painful vaccine for whooping cough or Painless? If the answer was limited only to the “pain ” factor of the vaccine, I would probably not even bother with this post.
But few things in life are that simple. Being parents mean, you have to take enormous ‘pains’ to make the right choice.The right choice and easy choice are not always best friends. With several types of research over the year, facts about the two types of DPT vaccine has become clear.
Though my aim is to help you take the right decision, you should always consult your pediatrician in case you have difficulty deciding.
What is Painless Vaccine?
The painless vaccine is the popular name of acellular Pertussis ( whooping cough) vaccine.It is generally given as a combination vaccine, DaPT (Diphtheria,acellular-Pertussis, Tetanus).
What is Painful Vaccine?
The painful vaccine is also Pertussis vaccine like the painless vaccine, but it is whole cell vaccine.This too is given as a DwPT combination. So , both these vaccines are Pertussis vaccines, with a few important differentiating factors.
Pros of Painless vaccine for whooping cough
Post-vaccination side effects, like high fever, persistent cry, local site painful swelling, are less common, in this variety than a Painful vaccine.
If a Painless vaccine is available, why the necessity of a Painful vaccine?
Experience with the Painless vaccine in industrialized countries has unfortunately brought forward two important facts:
1) Painful, vaccine (whole-cell) for DPT: Better priming, which means initial sensitization to the antigen (bacteria) is better. Thus antibody production is more and protection is superior.
2) The painless vaccine has a faster period of waning that is the immunity provided by the painless vaccine decreases faster than the painful vaccine. These two reasons are responsible for recent recurrence of whooping cough in many of the countries using only the Painless vaccine.
Why do we need to vaccinate our children against Pertussis?
Pertussis/Whooping cough/Kali Khasi/100 days cough are few of the names of the disease that can occur in all age groups depending on the level of immunity.
It is a bacterial infection of the respiratory tract. Highly contagious, usually manifesting as severe, persistent disturbing cough, lasting for a prolonged period not responsive to antibiotics.
Primary prevention (vaccination), and supportive care during disease are the management protocols. Infants are more at risk of debilitating course of the disease and thus most countries include this vaccine in their immunization schedule.
Current Recommendation for Pertussis vaccine
Indian Academy of Pediatrics recommends DPT vaccination.Give whichever is available. Importance of timely immunization cannot be stressed enough.
Primary immunization: at 6 weeks, 10 weeks and 14 weeks.
Booster dose: at one and a half years and five years.
When can you give Painless vaccine for whooping cough
- History of severe adverse effect to the previous dose of painful vaccine.
- A child having any neurological problem.
- Booster doses at one and a half years and five years.
Best practice:(IAP 2018 December Update)
The painful vaccine is to be preferred in all cases as it gives better protection for a longer duration. A healthy child is a happy child. As parents, we have to make choices which have the potential to change their lives forever. S
The Primary series or first three doses of DPT vaccine can be given with either wP(painful) or aP(painless vaccines). The painful vaccine is definitely superior (as recommended by WHO )to aP vaccine when it comes to level and duration of immunity. But causes a more local, painful reaction. In view of parental anxiety and concern regarding the local and systemic reactions to painful vaccines, the painless vaccine can also be given even in the primary series.

Things to Remember During Vaccination
What is Immunization?
It is a process of protecting from a certain disease. There are two ways to provide immunization – vaccination or naturally contracting a disease and then recovering from it. A vaccine when administered provides immunity against a disease. It can either be given through an injection or orally.
There are some vaccines which The Indian Government gives to everyone throughout the country in Immunization clinics.( BCG, Hepatitis B, OPV, IPV, DPT, Hib, Japanese Encephalitis, MR vaccine)
There are some which are yet to be implemented by The Government throughout the country but recommended by Indian Academy of Pediatrics as desirable vaccines for every child( Rotavirus vaccine, Pneumococcal vaccine, Typhoid vaccine, Hepatitis A vaccine, Chicken pox vaccine, MMR vaccine, Flu vaccine).

List of all Vaccines
- BCG (Bacillus Calmette-Guerin)
Bacillus Calmette-Guerin (BCG) vaccine is used against tuberculosis. It also prevents TB meningitis in children. Just like other vaccines, BCG contains a live attenuated culture of the Tuberculosis causing bacterium. It is in a weakened form. This means the disease-causing power of the bacteria is removed, but the antigenic property helps in triggering the immune system to get ready.This help protect the child from serious Tuberculosis infection in the future, but not from some of the manifestations of the disease.
Dosage – One dose through injection at birth
- IPV (Inactivated Polio Vaccine)
IPV vaccine is administered to prevent polio in the child, and it is a mandatory vaccine for all babies. An inactivated wild strain of poliovirus is given to children which leads to the production of antibodies. It is typically given with the DTP vaccine.
Dosage – Four doses of polio vaccine (IPV) intramuscular injection, though presently Government gives two doses of intradermal injections
- OPV (Oral Polio Vaccine)
Oral Polio Vaccine is given orally (OPV). It is made up of an activated non-wild strain of the virus that produces antibodies within the body.
Dosage – Three doses of OPV
- Hepatitis A
It protects against the Hepatitis A virus. Hepatitis A virus causes viral hepatitis and liver failure in children.Since this is easily spread through water and food ,protection is much needed.
Dosage – Two or one doses (depending on the type of vaccine).
- Hepatitis B
Hepatitis B vaccine helps prevent liver diseases caused by the virus of the same name. The virus can cause liver diseases such as liver cancer, liver failure, and even death.
Dosage – Three or four doses of vaccine depending on schedule
Prevents baby against Diphtheria, Tetanus, and Pertussis (whooping cough). It is a mandatory vaccine.
Dosage – Three doses, followed by 2 booster doses
This baby vaccination protects children against rotavirus that can cause severe diarrhoea.
Dosage – Three or 2 doses of vaccine depending on the type.
- HiB (Hemophilus Influenza type B)
Protects children against a bacteria called Hemophilus influenza type B – that causes diseases like pneumonia, meningitis, and epiglottitis.
Dosage – Three doses and a booster dose
- PCV (Pneumococcal Conjugate Vaccine)
Prevents against bacteria that cause pneumonia of lungs, brain meningitis, infection of blood, and ear infections.
Dosage – Three doses and a booster dose.
It protects children against measles, mumps, and rubella.
Dosage – Two doses of vaccine
- TCV (Typhoid)
Prevents typhoid which is a very common infection in India
Dosage – One dose of the conjugate vaccine of every 3 yearly dose of polysaccharide vaccine.
Why is Vaccination Mandatory?
Vaccination is mandatory because it is the best available method to prevent a deadly disease. It may hurt mothers to see their newborn undergo pain and it surely can ache their heart – however, vaccination is of critical importance to the baby’s health. It’s also important for long term protection against several conditions with very high mortality and life long disability.
Things to Remember During Vaccination
- Always follow the recommended schedule and get your child vaccinated as due. Set up a reminder and try not to miss the date prescribed by the pediatrician.
- Communicate with the pediatrician and be informed about what to do in situations such as fever diarrhea and cough. Vaccinations are typically not administered if a child has temperature or
- After a dose of vaccine, your child may get a fever that can last for two days. If the fever doesn’t go in two days, see the pediatrician
- Involving a couple of family members during vaccination can help distract the child and make the process easier.
- Be informed about painless vs painful vaccine.
Conclusion
Vaccination is a boon and a marvel of medical sciences. It protects us from critical illnesses and increases life expectancy. Today, vaccination is mandatory across the world. If you are pregnant, it is essential to know all that you can about vaccination and its impact on your child. There are some optional vaccinations too that one should consider – especially in today’s world when infections are so prevalent.

Swine flu (H1N1) in India is cause for worry due to the virulent nature of the virus. The public panic and terror about the disease are partly justified. Lack of awareness and extreme challenges of collecting data for surveillance has focused the world’s eye on India’s battle with the beast “swine flu”.
With the infection spreading by sneezing, coughing and close contact and the virus randomly combining with bird and pig influenza virus to produce new virus strains, it’s a science-fiction horror story unfolding at a rapid pace.
Swine Flu: Why do you catch it?
- Unusual prolongation of the winter season
- Lack of enough authorized testing centers
- Failure to rope in private-sector health care facilities adequately to handle the emerging epidemic
- Limited timely availability even in metro cities and Government setup
- Lack of Public awareness regarding the disease, vaccination, and treatment
- Possible major antigenic change in flu virus genetic makeup
Swine Flu Prevention
- Flu vaccine to people at risk urgently
- Dedicated Flu OPDS at all government and private health care centers.
- Reservation of Isolation beds for Swine flu cases at all government and private hospitals
- Increased number of Private and State-run testing centers
- Improved availability of flu drugs
- Flu drugs are given only with a prescription
- Public Health awareness drives to promote awareness, prevention, testing, and treatment.
- Maintain hand hygiene and personal precautions.
Symptoms of swine flu are similar to the common cold. But there is an increased risk of complications, especially in the very old and very young.
Swine Flu Symptoms: What happens if you have swine flu?
- High-grade Fever with chills and rigor
- Muscle pain
- Cough
- Nasal Secretions
- Fatigue
- Headache
- Bodyache
- Sore throat
- Nausea, Vomiting, dizziness, diarrhea
- Respiratory distress
- Need for intensive care in severe cases
Treatment of Swine Flu (H1N1)
The best treatment is prevention.
- Vaccination
- Personal protection, preventive care
- Drugs
- Supportive care
Swine Flu Vaccination
Who needs it: Risk group
According to CDC Recommendation, it is best practice to vaccinate the following groups of patients
- Children 6 months to 59 months/4 years and 11 months.
- Adults > 50 years.
- Adults and children with chronic pulmonary, cardiovascular, renal, hepatic, metabolic, hematologic, disease.
- Immunosuppressive patients including HIV.
- Women of reproductive age group planning pregnancy in this period and pregnant women.
- 6 months to 18-year children who are receiving long-term aspirin therapy.
- All Healthcare professionals.
- Household contacts and caregivers of people at high risk for complicated influenza, and caregivers of children. younger than 5 years and especially less than 6 months.
- Residents of chronic care facilities.
- American Indians, Alaskan natives.
- Morbidly obese people.
What is a flu shot?
- Intramuscular vaccine(given in the arm muscle with a syringe and needle)

- Trivalent (commonly), this means it will contain antigens (infection causing protein particles), from the three most common strains of virus causing influenza. Generally, it will have antigens belonging to Type A subtype H1N1, Type A subtype H3N2, and Type B virus antigens.
- Now Quadrivalent vaccine (Type A subtype H1N1, Type A subtype H3N2 and type B Victoria lineage and Type B Yamagata lineage, is also available in India.
- Composition: killed vaccine.As the virus in the vaccine is killed it will not cause swine flu in any patients.
Contraindication to flu vaccine:
1)Anybody with an allergy to eggs. Flu virus particles are cultivated in egg cells and are not given to people with egg allergy as it can cause a severe allergic reaction.
2) Previous history of severe reaction to the flu vaccine.
3) Anybody with moderate to severe illness with a fever should wait at least till fever subsides.
4) History of GBS (Guillain-Barré Syndrome, a condition where your bodies immunity may attack its own nervous system
The dose of vaccine :
0.25ml for 6 months to 3 years
0.50ml for more than 3 years
Timing:
It is a yearly vaccine and needs to be given every year before flu season. Preferably October onwards, and as soon as feasible. It takes 2 weeks for an adequate level of protection to be reached and you are at risk during this period.
Whether your child will need one or two doses will be best advised by your doctor.
But CDC recommends:
6 months to 8 years: 2 doses of vaccine > 4 weeks apart during their first year of flu season.
The first dose should be offered as part of routine vaccination to avoid missing the chance to provide adequate timely protection.
Opportunity for vaccination should ideally be given to all children as long as flu season persists.
Complications of Swine Flu vaccine
They include soreness, minor swelling, and redness at vaccine site, low-grade fever, nausea.
What is the vaccine efficacy against the current swine flu virus?
Influenza vaccines are intended to give protection against the most likely flu viruses (as determined by scientific research), which might cause influenza during the next flu season.
Shockingly CDC says the vaccine efficacy keep changing.That’s a big worry.The reason being that 70% of the circulating H3N2 viruses have major genetic variations (antigenic drift) which are different from the virus strain in the vaccine.
But you still need to vaccinate because
- Prevents infection and complications, from the circulating H3N2 Virus which might not have changed.
- May protect against other influenza virus infection like H1N1 and Influenza B
Can you get swine flu even after vaccination
Unfortunately yes.It’s possible as:
- You might be infected shortly before vaccination or within the time period it takes for adequate immunity to develop(generally it takes at least 2 weeks to be properly protected, and the protection gradually becomes less as the time following vaccination increases.)
- You might be infected with an influenza virus not included in the Trivalent vaccine (There are many varieties of the flu virus, the vaccine includes the most common ones causing flu)
- People who have chronic diseases, very young children, and old people may not be totally protected. Some of them may still be infected with the influenza virus included in the vaccine.
This occurs as the protection provided by the flu shot, is best in healthy young adults and older children. Protection varies with age, general health, and other coexistent diseases.
Nasal spray vaccine
Not popular in India. It has the following features:
- Live virus
- An intranasal spray containing the virus sprayed into the nasal cavity
- Recommended in 2 years to 49 years, healthy people.
- Single Dose
- Composition the same as that of an intramuscular flu shot
- A similar level of protection
Contraindications of nasal spray vaccine (as recommended by CDC)
- People with depressed immunity
- Having egg allergy
- Previous history of GBS syndrome (Guiallane -Barre syndrome)
- Children <2 years and more than 49 years
- Children 2 years to 17 years taking aspirin.
- History of antiviral drugs being taken orally within the last 48 hours.
- Past history of allergy to flu vaccine /its components.
- History of chronic respiratory disease like asthma, with an episode of wheezing within the last 12 months
- Pregnant women
Conclusion
India has a population of 1.27 billion with most cities being overcrowded. What works in western countries may largely not work here, due to the socio-economic, cultural and educational background.
But whatever our drawbacks, if you can follow the personal care precautions and get vaccinated you are halfway prepared to fight the disease. Rest is God’s will. Vaccination and personal safety precautions are still our best chance of protection.

According to the data available with the Union Health Ministry, 2167 unfortunate people have lost their lives to this disease.35077 people have fallen ill with Swine flu till date.Gujrat has been the worst affected with the death toll being 467 according to The Hindu.
Even though Delhi and Tamil Nadu both have been reporting a high number of cases, the death toll was not as high due to better public awareness and healthcare infrastructure.
Outbreaks of Swine flu had started since late 2014 in India. Increased ability of the H1N1 virus to cause infection, has been forefront among the many reasons for such a prolonged and severe Flu season.
MIT researchers have claimed that the Indian strain is a more virulent variety of H1NI.A mutation causing changes in the Haemagglutinin protein has been cited as a probable cause of increased virulence.Indian researchers have however challenged this claim by saying that their research doesn’t show any such variation in the genetic nature of H1N1.
Update May 2017
Swine flu has claimed 160 lives in various parts of the country till March. Minister of State for Health Anupriya Patel in a written reply in Lok Sabha said between January 1 to March 26, 160 people died from swine flu, while 6,062 cases were registered(New Indian Express)
A total of 844 cases reported in Maharashtra DNA NEWS.
H1N1 influenza has claimed 23 lives in Kerala so far in 2017, a total of 300-400 swine flu.(DNA news). In 2016, more than 12 lakh children less
In 2016, more than 12 lakh children less than five died in India. Of these deaths, pneumonia caused more than 1.7 lakh deaths and ” Dr. VK Paul, Department of Pediatrics, AIIMS.(DNA news).That’s the problem with swine flu in children.They just can’t cope.

- A total number of swine flu positive patients within VMC limits is 400. 242 have been cured, 17 have died the condition of 133 patients is stable, eight patients are on a ventilator.
- Maharashtra stats (source):414 deaths and 4,014 cases.Maharashtra reported 50% of all H1N1 influenza casualties and one-fourth of the confirmed cases in India since January.
- 280 suspected cases of swine flu have been reported in Punjab.75 cases were positive and 15 mortality.
- H1N1 increased three times this year.18,000 affected. 871 deaths across India so far.Last year 1,786 cases and 265 deaths.

Super tips to Ace Super Blogger Challenge 2019

From next week onwards it will open every Thursday at 12.01 am (midnight)
Super Blogger Challenge 2019 Quick rules (Read Full Terms and Conditions)
This will run for 4 weeks that is from 12:01 AM IST on 7th February 2019 to 11:59 PM IST on 7th March 2019.
- One week will be count from Thursday to Wednesday.
- You have to write one Blog post every week. You can choose from our Topic ideas or can take your own ideas.
- You can draft your post with given Blog post content ideas or as per your wish. Please notice you must write every week and link up on time to be eligible for the prizes.
We will be giving you a sudden prompt too in the 2nd week. That is to test your response to sudden writing events you may have an opportunity to write for.
